This agreement formalizes your voluntary role with SFRD. It is designed to protect you, the foundation, and most importantly, the patients and families we serve. Please read each point carefully.

I understand and acknowledge that my role as a volunteer for SFRD is entirely pro-bono (unpaid). I consent to SFRD securely storing the personal information I have provided in this form in its internal, confidential volunteer database for the purpose of coordination and communication. Confidentiality Agreement: I understand that in my role, I may be exposed to sensitive patient information. I agree to maintain the strictest confidentiality of all patient and family information and will not share it with any third party, in accordance with SFRD's privacy policy.
